Does this only work for recently uploaded images?

No.
Is it possible to apply a watermark to old images (already saved)?
No.

Yes and no. It depends what happens.
Can a watermark be placed anywhere in the image?
No. It will only be added to the specified most common areas of the image (see screenshot for details).
Will the watermark also be added to the thumb?
Yes.
